Operation Understanding Grant Application

Please answer the following questions briefly but completely.  Your grant application will not be considered without all of the requested/required information.  If you need assistance or have questions about the application, please contact Gerry Garber, the Operation Understanding Chair, via email or phone at (717) 798-1301.

The deadline for the next grant cycle is August 31, 2017.

Is this your first request for ministry funding? If not, when did you receive funding in the past? If this request is for “ongoing” (not initial) funding, explain the need for continued financial assistance. Also, have you received (or expect to receive) other sources for funding? If so, include the following information in your grant application request: - List the different sources of funding and the amounts received and pending receipt. - List the sources and amounts of funding you expect to seek in the future.
Also, furnish the mailing address and daytime telephone number of the ministry’s principal contact person. If available, include an email address. You should include any additional information about your ministry that you feel will be useful for the OU Committee to know when they discuss your grant application.
Files must be less than 2 MB.

Grant applications are sent electronically to the Diocese by clicking on “SUBMIT” at the bottom of the attached form. Grant applications are due by October 31, 2016.  If you do not have internet access, you may send a paper copy of the application to Gerry Garber at 550 Harvest Drive, Harrisburg, Pa. 17111 or call (717) 798-1301 if have questions or need assistance when completing the form.
